Chicago Century Furniture Corp. Recalls Mattresses for Violation of Federal Mattress Flammability Standard Sold Exclusively at Chicago Century Furniture

Hazard: The mattresses fail to meet the mandatory federal open flame standard for mattresses, posing a fire hazard to consumers.

Incidents/Injuries: None reported.

Description: This recall involves Chicago Century Furniture Corp. twin-, full-, queen- and king-size mattresses with the model number “MG006.” The mattresses are 3 inches thick with white and off-white fabric with a tone-on-tone swirl pattern. The model number can be found on a paper tag attached to the foot of the mattress. The words “Century Spring Mattress” appear on a fabric label on the topside near the foot of the mattress. A picture of a man and woman embracing also appear on the label.

Sold exclusively at: Chicago Century Furniture Inc. 3034 South Wentworth Ave., Chicago, Ill., from January 2008 through October 2010 for between $190 and $360.

Manufactured in: China

Remedy: Consumers should immediately stop using the mattress and contact Chicago Century Furniture Corp. to receive a replacement mattress.

Clip-on Desk Lamps Sold At Lowe's Stores Recalled Due to Shock Hazard; Made by He Shan Lide

Hazard: The power cord for the lamp can detach where it meets the clamp, exposing energized wires, posing an electric shock risk to consumers.

Incidents/Injuries: The firm has received five reports of cords detaching, including one reported electric shock and one report of a child receiving burns to her hand and leg.

Description: This recall conducted by He Shan Lide involves pink and blue plastic LED desk lamps with a flexible metal neck approximately 12" long and a clamp at the base. The power cord has a silver label attached to it. On one side of the label "MET," "Electrical Safety", "E113152" and "Apr. 2011" are printed. The other side of the label includes the model number F3044-AC-01 (pink lamps) or F3044-AC-02 (blue lamps). The plug on the power cord is labeled "KENIC KE 01P E155176."

Sold exclusively at: Lowe's stores nationwide from May 2011 to December 2011 for about $20.

Manufactured in: China

Remedy: Consumers should immediately stop using the lamp, unplug the power cord, cut the power cord off 2” to 3” above the plug, discard the lamp and send the plug to He Shan Lide, 4570 Eucalyptus Ave, Suite C, Chino, CA 91710 along with their name and address to receive a full refund, including the cost of postage for returning the plug.

Summary:
Toyota is recalling certain model year 2013 Lexus GS350 vehicles, manufactured from December 12, 2011, through February 7, 2012. The steering control electronic control unit (ECU) programming for the variable gear ratio steering (VGRS) system could cause the steering wheel to become off-centered when restarting immediately after the vehicle is parked wit the steering wheel turned.

Consequence:
In this occurs, the driver may not realize that the steering wheel is off-centered and the vehicle could maneuver in an unexpected direction, resulting in a crash.

Remedy:
Toyota will notify owners, and Lexus dealers will update the VGRS system free of charge. The safety recall is expected to begin in early June 2012. Owners may contact Toyota at 1-800-331-4331.

Nevada Firm Recalls Lasagna Product Due to Misbranding and an Undeclared Allergen

West Best Foods, Inc., a Las Vegas, Nev. establishment, is recalling approximately 13,776 pounds of lasagna products because of misbranding and an undeclared allergen. The product contains soy, which is an allergen that is not declared on the label, the U.S. Department of Agriculture's Food Safety and Inspection Service (FSIS) announced today.

The products subject to recall are: [View Labels (PDF Only)]

  • 24-lb cases of "WEST BEST FOODS INC LASAGNA WITH MEAT SAUCE."
  • 24-lb cases of "WEST BEST FOODS INC DELUXE BEEF LASAGNA."


The products bear the establishment number "EST. 6080" inside the USDA mark of inspection. The products were produced between April 13, 2012, and May 21, 2012, and were distributed to hotels and one distributor in Las Vegas, Nev.

The problem was discovered during a routine food safety assessment and may have occurred as a result of a change in an ingredient formulation used in the products. FSIS and the company have not received reports of adverse reactions due to consumption of these products. Anyone concerned about a reaction should contact a healthcare provider.

FSIS routinely conducts recall effectiveness checks to verify recalling firms notify their customers of the recall and that steps are taken to make certain that the product is no longer available to consumers.

Salinas Firm Initiates A Voluntary Recall Because Of Possible Health Risk

Taylor Farms Retail, Inc. is initiating a voluntary recall of Organic Baby Spinach with the potential to be contaminated with Salmonella following a random test conducted on a finished package of spinach by USDA.

Salmonella is an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Healthy people may experience fever, nausea, vomiting, diarrhea (which may be bloody), and abdominal pain. In rare cases the organism can get into the bloodstream and cause more serious complications.

There have been no reported illnesses attributed to the recalled items. The “Best By” Code Date for the recalled products can be located on the lower right portion of the front label. These recalled items were distributed from Taylor Farms Retail, Inc. on May 9, 2012 and May 10, 2012.

No other products or code dates are affected by this recall. The company is cooperating with the Food and Drug Administration (FDA) and California Department of Public Health (CDPH) regarding this recall.

Customers who have purchased these products are urged not to consume the products and should dispose of them. Pacific Coast Fruit Company Recalls Single Side Serving Processed Salads for Potential Listeria monocytogenes Contamination

Pacific Coast Fruit Company, Portland, Oregon is voluntarily recalling multiple types of bagged processed salads based on the potential contamination of Listeria monocytogenes, an organism which can cause serious and sometimes fatal infections in young children, frail or elderly people, and others with weakened immune systems. Although healthy individuals may suffer only short-term symptoms such as high fever, severe headache, stiffness, nausea, abdominal pain and diarrhea, Listeria infection can cause miscarriages and stillbirths among pregnant women. 

Pacific Coast Fruit utilized a portion of the salads that were recalled by River Ranch in some of our retail and food service processed bagged salad.  Please see attached list for specific products entered into commerce, including UPC code, Pacific Coast Fruit Item Number, and Best if Use By Date or Production Date.

Pacific Coast recalled products were distributed to Fred Meyer, Avanti, and Evergreen in Oregon, Washington, Alaska and Idaho.

The recalled retail salads are packed in a 9.6oz -10oz clamshell style and VA food service items is packed in 5 lbs – 20 lbs food service salads bags.

There have been NO reported illnesses associated with this recall.  This voluntary recall is being conducted with the knowledge of the U.S. Food and Drug Administration.

Consumers who have purchased this product should not consume it and are urged to return it to the place of purchase or destroy the product.  Consumer with questions can contact Pacific Coast Fruit at 503-234-6411 between the hours of 8 am-4:30 pm PST Monday to Friday.

Bicycle Stems Recalled by Ridley Bikes Due to Fall Hazard

Hazard: The bicycle handlebar stems can crack or break, posing a fall hazard.

Incidents/Injuries: The firm received one report of a handlebar stem breaking resulting in minor injuries.

Description: The recall involves all 4ZA 31.8 threadless bicycle handlebar stems sold as individual aftermarket components. The stems are black and have the word "4ZA Cycling Performance" painted on the carbon extension between the steerer clamp and handlebar clamp. The recalled stems range in extension lengths from 90mm to 130mm, approximately 3.5 to 5 inches.

Sold at: The stems were sold at specialty bicycle retailers and via websites from January 2005 through April 2010 for about $59.

Manufactured in: Taiwan

Remedy: Consumers should immediately stop using their bicycles and contact the bicycle retailers where the stems were purchased to obtain a free replacement stem. Other bicycle shops can handle the replacement process by contacting the manufacturer's current distributor, Quality Bicycle Products, Inc.

Summary:
Chrysler is recalling certain model year 2012 Jeep Wrangler vehicles equipped with an automatic transmission. The transmission skid plate, which is close to the catalytic converter, can collect debris.

Consequence:
If this occurs, the debris can ignite without warning, resulting in a vehicle fire.

Remedy:
Chrysler will notify owners, and dealers will replace the skid plate with a skid bar, free of charge. The safety recall is expected to begin during May 2012. Owners may contact Chrysler at 1-800-853-1403.

Minnesota Firm Recalls Steakhouse Burgers Due to Misbranding and Undeclared Allergens

J&B Group, a Pipestone, Minn. establishment, is recalling approximately 456 pounds of steakhouse burgers because of misbranding and undeclared allergens. The product contains a seasoning mix with hydrolyzed soy and wheat proteins, known allergens, not declared on the label.

  • 27-oz packages containing 6 4.5-oz. "No Name, Roasted Peppers, Onions and Mozzarella Cheese Steakhouse Burgers" with a Julian date of "03312."
  • Shipping cases of "Beef Steakburger with Steakhouse Seasoning" with serial numbers of "1049XXX" a case code of "19051" and the Julian date "03312."


The product bears the establishment number "Est. 8934" inside the USDA mark of inspection. The products were packaged on February 3, 2012 and were distributed to retail outlets in Minnesota and Wisconsin.

The packages labeled "No Name, Roasted Peppers, Onions and Mozzarella Cheese" were incorrectly packed and instead contain a "Steakhouse Burger." The "Steakhouse Burger" product in the package includes a seasoning mix that contains soy and wheat proteins, known allergens. Soy and wheat are not included in the product's ingredient list on the label. FSIS and the company have not received any reports of adverse reactions due to consumption of the products. Gills Onions Voluntarily Recalls One Lot of Diced Red Onions Because of Possible Health Risk

Gills Onions, LLC of Oxnard, CA is initiating a voluntary recall, 2,360 pounds, of diced red onions with lot code 51RDA1A2119 and use-by-dates May 14, 15 and 17, 2012 because it may be contaminated by Listeria monocytogenes. The product is beyond its use-by-date, no illnesses have been reported in connection with this recall, and no other Gills Onions product is affected by this recall.

The recalled diced red onions were distributed directly from Gills Onions to retailers in Canada and retailers and foodservice distributors in California, Oregon, Washington, Arizona, Idaho, Texas, Illinois, Michigan, Arkansas, Ohio, Tennessee, New Jersey, Georgia, and Florida.

Recalled product sold at retail was packaged in a 7oz. clear plastic clamshell, labeled Gills Onions with the May 17, 2012 use-by-date and the lot code 51RDA1A2119 stamped on the UPC label. The UPC number in Canada is 643550 00045 0 and the UPC number in the United States is 643550 00041 2. However, since this number can also be on other products, please refer to the lot code in order to identify the recalled product. Photos are available on our website. Anyone who has the recalled product in their possession should discard it.
Product sold to foodservice customers was packaged in 5 lb clear plastic bags, identified as Gills Onions Diced Red Onions, and stamped with an expiration date of May 14 or 15, 2012. The lot code 51RDA1A2119 is also stamped on both the bag and box. Photos are available on our website.

This voluntary recall was initiated as a result of a routine and random test directed by the Canadian Food Inspection Agency. Gills Onions has notified all customers who received the recalled lot directly from the company and requested that they remove from commerce any of the expired product they may still have in stock. As an extra precautionary measure and because we do not always know to whom our direct customers sell our products, we are also asking our direct customers to notify their customers of this recall. Gills Onions is keeping the U.S. Food and Drug Administration and the Canadian Food Inspection Agency informed of its recall process to assure that consumers are properly alerted.
